Triple square drives are used in high torque applications, such as cylinder head bolts and drive train components. Triple square fasteners are commonly found on German vehicles such as Audi, BMW, Mercedes, Porsche, and Volkswagen.

Easiest way I’ve found is to use a left hand drill bit. Get one a size or two smaller than the minor thread of the bolt so you don’t hit the threads in the flange. Put the cordless drill in reverse and the cutting action of the drill bit helps back the bolt out.
